General Accreditation Requirements
- Membership in the New York State Council on Divorce Mediation
- Submission of $75.00 application fee. Checks should be, made out to NYS Council on Divorce Mediation
- Submission of ONE Proof of Requirement (click here to read the different routes accepted)
- Submission of cover sheet of your malpractice insurance
- Proof of Advanced Practitioner Membership in ACR, if applicable
- Proof of additional ten hours of consultation, if applicable (click here to read consultation requirements)
- Submission of three (3) copies of six (6) representative Memoranda of Understanding, if applicable
